What do you need to bring?
Please bring a valid government-issued photo ID along with your completed Live Scan Request form to the office listed below.
A completed fingerprint authorization form is required for all fingerprinting services.
*Expired ID's will not be accepted*
Please contact your requesting agency to obtain the correct form number (the general-purpose form number is BCIA 8016). Forms for certificates, licenses, permits, and registrations are available on the California Department of Justice website. The appropriate form must be fully completed and presented at the time of fingerprint processing.
