What to Bring
Prepare for your Live Scan appointment by bringing the following items.
Required Items
- Valid government-issued photo ID (Driver's License, State ID, Passport, or Military ID)
- Your completed Request for Live Scan Service form (ORI number required — provided by your requesting agency)
- Payment for the rolling fee ($50) and any applicable DOJ/FBI fees
- Any additional forms required by your requesting agency
